Forms, books and guidelines

This page lists forms, books and guidance documents that Defra, The Environment Agency and the Fish Health Inspectorate provide under the Aquatic Animal Health (England and Wales) Regulations 2009.

Forms

• RW1 form - Application to register a fishery
  
• RW2 form - Application to register an Aquatic Animal Holding
  
• AW1 form - Application to authorise or change an aquaculture production business (APB)
  

  The FR1, FR2 and CR1 are 'e-forms' that allow the user to type directly onto the form and save using Adobe Acrobat Reader. Use the 'Tab' key to move from field to field. The applicant must print and sign the application forms before sending to the Environment Agency, contact details given on the form.

• FR1 form - Application to introduce fish, fry or ova
• FR2 form - Application to use fishing instruments (other than rod and line) and/or remove fish from inland waters
• CR1 form - Application to trap and/or remove crayfish
• AAH1 - Notification to import live fish and shellfish into England and Wales from another EU territory
• AAH2 - Application to become an authorised aquaculture production business (APB) to import live fish and shellfish into England and Wales
• CVED notification - form to import live fish and shellfish from third countries
• CVED Timetable - notification to import live fish and shellfish from third countries
• CRAY2 - Application for a licence to keep non-native crayfish
• ILFA1 application - Licence to keep or release non-native species
